Records Management

The Regional Municipality of Durham, in its capacity as a delivery agent of Ontario Works, must observe the requirements of the “Freedom of Information and Protection of Privacy Act” (FIPPA) and the “Municipal Freedom of Information and Protection of Privacy Act” (MFIPPA) regarding the disclosure of information and personal privacy.

The Business Affairs and Financial Management Division maintains confidential records and sensitive information that Ontario Works staff uses in their day-to-day business. Staff ensures that measures are in place, so sensitive material is protected and held in a secure manner, as per the provincial legislative requirements. All staff is informed and educated regarding the standards that must be maintained, as are necessitated by their job function, and understand any applicable directives that apply to the handling, preparing and maintaining of such records.

T5 information

Every year a T5 document (known as the T5007 slip) is issued to identify recipients who have received social assistance payments, and the amount of the benefits paid to them. The T5 benefits are non-taxable, but recipients must include these amounts in their net income when filing their annual tax returns.

T5 information is sent on or before the last day of February, following the calendar year to which the information returns applies.

If you did not receive your T5 slip, you can obtain a replacement T5 by calling 905-666-6239 ext. 2511 (or toll-free at 1-800-372-1102 ext. 2511).

All T5 slips are replaced by the office where the client is currently receiving assistance or, for clients that are now terminated, the office where the client last received assistance.
