How to Complete an Income Reporting Statement (IRS)
When is the Income Reporting Statement (IRS) due?
The Income Reporting Statement (IRS) is due on the 16th day of each month. Submitting the IRS on time will help to avoid a delay in receiving your monthly financial assistance. It can either be mailed by postage paid-envelope, or dropped off at your office location. The offices are located at 605 Rossland Rd. E., Whitby, 505 Wentworth Street W., Oshawa, 140 Commercial Ave., Ajax, or 2 Campbell Dr.., Suite 305 Uxbridge. The IRS can also be faxed to your local office or dropped off at our Resource Centre in Oshawa located on the second floor at Midtown Mall, 200 John St. W., Oshawa. If you have a declared income we will also need a copy of your verification (such as pay stubs for the period of the declared income).
Submitting your IRS before this date is not recommended. Your caseworker may not accept this document early, and may return it to you to submit on the appropriate date.
Acceptable Income Reporting Statement Examples – When you have no income to declare
We will accept any of the following Income Reporting Statement (IRS) examples when you have declared NO INCOME. It is acceptable to be completed in pencil or pen and it must always be dated and signed.
Missing your Income Reporting Statement?
In the event that you have misplaced your original Income Statement we have provided you with a blank copy (PDF). It is important that you complete your full name and identify the 16th of what Month and Year on the blank copy. If you have a declared income we will also need a copy of your verification (such as pay stubs for the period of the declared income) submitted with this downloaded Income Reporting Statement (PDF).
