Sexual Health Resource Order Form

Resource orders are limited to health care professionals and community agencies located in Durham Region, Ontario, Canada who require information for patients and/or clients. All resources listed below are Durham District School Board (DDSB) approved. Area residents interested in obtaining any of the resources listed below are asked to call the Durham Health Connection Line at 1-800-841-2729. Durham Region Health Department may contact you regarding your order.
