Consent Forms

The school will give a consent package to your Grade 7 child at the beginning of the school year. Review this package with your child. Communicate your vaccine wishes by completing all three consent forms and return them to the school. If you require another copy of the consent package please ask your school secretary.

Missed a Dose at School?

If your child missed getting the vaccine(s) at school he/she may get it at their next school clinic, or you can call the Durham Health Connection Line at 905-666-6242 or 1-800-841-2729.

Multiple Vaccines

School-based immunization clinics provide up to three vaccines at one time. It is safe for your child to get multiple vaccines.

Don't forget to update your record of protection.

After you or your child receives a vaccine, update your immunization record (yellow card).

*Parents must contact the Health Department if you object to immunization based on conscious or religious beliefs, or if your child cannot be immunized for medical reasons
