Report a student’s immunizations

Every time a student gets immunized by a health care provider, parents and/or guardians must report it to Durham Region Health Department. Please attach a photo of the immunization record.

Please do not report immunizations received at school or clinics that were provided by Durham Region Health Department. These will be automatically entered by Health Department staff.

Book an immunization appointment

Durham Region Health Department will be offering immunization clinics for students attending elementary and high schools in Durham Region, who do not have OHIP coverage, a health care provider, or are experiencing difficulty getting vaccines though their health care provider.

  • Vaccines are free of charge.
  • Students must live in, or attend a school in Durham Region.
  • Book an immunization appointment online.

Students entering grades 7 and 8 in the 2023-2024 year will be offered school-based vaccines (Men-C-ACYW135, Hep B, and HPV-9) at school clinics in the fall and spring.

Vaccine laws in Ontario help protect children who attend licensed child care centres and schools.

Diseases can spread quickly in child care centres and schools. Vaccines help your child develop immunity to prevent disease and limit spread to others. Vaccines in Ontario's routine immunization schedule are provided free of charge if you meet the eligibility criteria.

Report immunizations online using the Immunization Connect Ontario (ICON) tool

Every time a school-aged child gets immunized by a health care provider, parents and/or guardians must report it to Durham Region Health Department.

You can view or report immunizations online using the Immunization Connect Ontario (ICON) tool. Please attach a photo of the record.

How to submit or view immunization records

To submit or view your child's immunization information online you need:

Option 1: Ontario Health Card (OHIP)

  • Immunization record or supporting documents.
  • Ontario Health Card.

Option 2: Ontario Immunization Identification ID (OIID) Number

  • Immunization record or supporting documents.
  • Ontario Immunization ID found on the top right-hand corner of the Durham Region Health Department immunization letter or notice.
  • Ontario Health Card number (OHIP): You only need this the first time to set up a unique PIN. If you don’t have OHIP, call us.
  • Unique PIN.

Disclaimer: Immunizations on file at the Health Department may only be as accurate as what we have received. Please submit any missing immunizations to ensure we have a complete record. Include photos of the records or supporting documents.

Note: It may take up to ten business days before any updates appear on your child’s record when you use Immunization Connect Ontario ICON.

If entering vaccine information online using ICON, please type the vaccine type as written below:

Two months of age (Three immunizations due)

  • DTaP-IPV-Hib | Diphtheria, tetanus, pertussis, polio, Haemophilus influenzae type b vaccine
  • Pneu-C | Pneumococcal conjugate 13 vaccine (e.g. Prevnar 13 ®)
  • Rot-5 | Rotavirus vaccine (e.g. RotaTeq ®)

Four months of age (Three immunizations due)

  • DTaP-IPV-Hib | Diphtheria, tetanus, pertussis, polio, Haemophilus influenzae type b vaccine

12 months of age (Three immunizations due)

  • MMR | Measles, mumps and rubella vaccine (e.g. Priorix ®, M-M-R II ®)
  • Men-C-C | Meningococcal conjugate type C vaccine (e.g. Menjugate ®, Meningitec ®, NeisVac-C ®)
  • Pneu-C | Pneumococcal conjugate 13 vaccine (e.g. Prevnar 13 ®)

Doses of MMR vaccine, Men-C-C vaccine and Pneu-C-13 vaccine are needed on or after age one regardless of previous doses.

15 months of age (One immunization due)

  • Var | Varicella (chickenpox) vaccine

18 months of age (One immunization due)

  • DTaP-IPV-Hib | Diphtheria, tetanus, pertussis, polio, Haemophilus influenzae type b vaccine

Four years of age (Two immunizations due)

  • Tdap-IPV | Tetanus, diphtheria, pertussis and polio vaccine (e.g. Adacel-Polio ®)
  • MMRV | Measles, mumps, rubella and varicella vaccine (e.g. Priorix-Tetra ®, ProQuad ®)

A dose of Tdap-IPV vaccine is needed on or after four years of age regardless of previous doses.

Grade 7 vaccines
(Report online ONLY if received somewhere other than a Durham Region Health Department clinic, e.g. school or catch-up)

  • Men-C-ACYW135 | Meningococcal conjugate type ACYW135 vaccine (e.g. Menveo ®, Menactra ®, Nimenrix ®)
  • HB | Hepatitis B (e.g. Recombivax ®, Engerix B ®)
  • HPV9 | Human papillomavirus (e.g. Gardasil 9 ®)

14 - 16 year old booster (ten year booster) - Most common missing vaccine for high school students

  • Tdap | Tetanus, diphtheria, pertussis vaccine (e.g. Adacel ®, Boostrix ®)
Other ways to report immunizations
  • Fax the record to the Durham Health Connection Line at 905-666-6216.
  • Mail a copy of the record to:

  • Hand in a copy of the record to your child care centre or school office.
  • Call the Durham Health Connection Line.
Requesting a copy of your child’s immunization record

The best way to access your child’s record is using the online tool Immunization Connect Ontario (ICON).

To view or print your child’s record on file with the Health Department using ICON, you will need to create an account and set a PIN with your child’s Ontario Immunization ID (OIID) and Ontario Health Card.

  • If you are not able to set up your PIN online, please call 1-800-841-2729 to get help creating your account.
  • You can also call the Durham Region Health Connection Line to request a copy of the immunization record.
Grade 7 and 8 school immunization clinics

The Health Department is providing school immunization clinics for eligible students in grades 7 and 8.

Students in grade 7 and 8 are eligible to for vaccination at one of our school-based clinics.  The vaccines are free of charge.  The following vaccines are being offered at the clinics currently:

  • Meningococcal-C-ACYW135 - required for school, typically given at students in grade 7 at school
  • Hepatitis B - recommended but not required for school, typically given to students in grade 7 at school
  • Human Papillomavirus - recommended but not required for school, typically given to students in grade 7 at school

Students who did not finish their vaccine series will only need to get the remaining dose(s). A vaccine series does not need to be restarted, even if the recommended time between doses has passed.

You can check if your child is up-to-date on their routine vaccinations through Immunize Connect Ontario (ICON).

If you received a vaccine through your primary care provider or family doctor, the ICON system is not automatically updated. Please update your child’s vaccine record in ICON if they received a Human Papilloma Virus (HPV), Hepatitis B (Hep B), Meningococcal C-ACYW135, or Tdap vaccine from a primary care provider or family doctor.

If your child received a vaccine through our school-based immunization clinic, update your immunization record (yellow card). You do not need to update in ICON.

About the grade 7/8 school immunization program

Durham Region Health Department gives up to three vaccines to grade 7 students. Due to the COVID-19 pandemic, routine immunization in schools will also be offered to eligible grade 8 students.

The Health Department will be offering Human Papilloma Virus (HPV), Hepatitis B (Hep B), and Meningococcal C-ACYW135 vaccines at our school-based immunization clinics.

  • Human Papillomavirus (HPV) (2 doses, 6 months apart)
  • Hepatitis B (2 doses, 6 months apart)
  • Meningococcal C-ACYW135 (Men C-ACYW135) (one dose only)

Consent forms
The school will give a consent package to your grade 7 and grade 8 child at the beginning of the school year. Review this package with your child. Complete the consent form and return to the school. If you require another copy of the consent package please ask your school secretary.

Prepare your child for getting a vaccination

Before the vaccination, let your child know:

  • What to expect.
  • Why the vaccines are important.
  • That the needle might feel like a pinch in the upper arm that only lasts a few seconds.
  • That the nurse will help to decrease their fear/pain (e.g.: distraction, deep breathing, relaxing their arm, talking them through the experience etc.).

On the day of the vaccination, have your child:

  • Eat something before their appointment.
  • Wear a shirt with loose sleeves.
  • Wear a mask to access the clinic.
  • Follow instructions provided by the public health nurse and your reminder email to access the clinic.

If your child is nervous about getting needles, use the CARD system to have a more positive vaccination experience.

Missed a dose at school?

If your child missed getting the vaccine(s) at school he/she may get it at their next school-based clinic. If you have questions, you can call the Durham Health Connection Line.

Immunization reminder letters

Every year, the Health Department reviews immunization records of students to ensure they are up-to-date for required vaccines. Our goal is to ensure students are immunized to help keep them healthy and avoid the spread of vaccine preventable diseases, while meeting requirements to avoid future suspension. Health care providers are not required to update the Health Department with vaccines your child has received. We rely on parents to keep us updated.

Students will not be at risk for suspension for overdue immunizations for the 2022-2023 school year.

If you received an immunization reminder letter from us, we are missing vaccine information for your child.

Here is what you should do next:

  • Talk to your health care provider to find out if your child is up to date.
  • Compare the record on the letter with your health care provider’s record.
  • If your child needs vaccines, see if you can make an appointment with your health care provider. You can also make a plan with your health care provider to receive any missing vaccines in the future. If you don’t have one, a health care provider, try calling walk-in clinics in your area as some of them may be able to give your child the missing vaccines. Bring the letter with you to your appointment.
  • Give us the update for the missing vaccines.
  • This year, Health Department nurses will be going to schools in February and March to offer all routine vaccines. This will also include the recommended vaccines, hepatitis B and HPV9, which are not required by the Immunization School Pupils Act (ISPA).

You may be able to view your child’s vaccine record online. You can also call us for a copy.

Vaccines needed for school

All students under 18 years of age need vaccinations for the following diseases, as required by the Immunization of School Pupils Act (ISPA):

  • Tetanus
  • Diphtheria
  • Pertussis
  • Polio
  • Measles
  • Mumps
  • Rubella
  • Varicella (if born on or after January 1, 2010)
  • Meningococcal disease group C (under 12 years of age)
  • Meningococcal disease group ACYW-135 (over 12 years of age)

Immunization exemptions

Medical exemptions

If your child cannot get vaccines due to medical reasons, download the Statement of Medical Exemption - ISPA form. This form must be completed by a doctor or nurse practitioner.

Conscience or religious exemptions

Durham Region Health Department is continuing to work with the Ministry of Health on the enforcement of mandatory vaccines under the Immunization of School Pupils Act (ISPA). School suspensions under ISPA will not be enforced during the 2022/2023 school year. Vaccine education sessions are currently not available, as a result the Health Department is not currently accepting submissions for non-medical exemptions. Please continue to check back for future updates.

Vaccines required for child care (under the Child Care and Early Years Act)

Children who attend a licensed child care centre in Durham Region need valid vaccinations for the following diseases:

  • tetanus
  • diphtheria
  • pertussis
  • polio
  • Haemophilus influenzae type b
  • measles
  • mumps
  • rubella
  • varicella
  • meningococcal disease group C
  • rotavirus
  • pneumococcal disease

Immunization exemptions

Children who cannot get vaccines due to medical, conscience or religious beliefs can download one of the following exemption forms, specific to the Child Care and Early Years Act (CCEYA):

  • Statement of Medical Exemption - CCEYA
  • Statement of Conscience or Religious Belief Affidavit - CCEYA (signed by a Commissioner of Oaths, such as a lawyer, Member of Parliament or Town Clerk)

You must provide the original form to the child care centre.
