Coordinated Access is Durham Region’s community-wide approach to connecting individuals experiencing or at risk of homelessness with appropriate housing and support services. It ensures that people are matched to the right supports at the right time, based on their needs and circumstances.

Coordinated Access and Durham's By-Name List

By-Name List (BNL)

The By-Name List is a real-time, up-to-date list of individuals known to be experiencing homelessness. It allows communities to understand individual's unique needs to match them to appropriate housing resources. This list is generated through Coordinated Access.

If you are facing homelessness, you can be connected to the following Community Access Points, in order to complete a vulnerability assessment and be added to the Durham Region By-Name List.
