Long-Term Care Homes

The Regional Municipality of Durham's four long-term care homes are accredited. The Ministry's Fixing Long-Term Care Act (FLTCA), 2021 regulates them.

Each home area has its own dining room, lounge area, activity room, nurses' station, and balcony overlooking our landscape and gardens.

We encourage regular visits from family and friends. Family and friends are welcome to participate in our social programs, Family Council and resident care conferences. There are no restricted visiting hours. However, we ask visitors to check with staff if they want to visit between 8 p.m. and 7 a.m.

Seaton Village is a new long-term care home, owned and operated by The Regional Municipality of Durham, that is projected to open in late 2026. This 200-bed home is located in the Seaton community, between Brock Road and Whites Road in Pickering. The home has a dementia-friendly design, 24/7 nursing care and offers social spaces, a store, hair salon, bank, worship centre and fitness area.

Services

Seaton Village will offer a wide range of specialized services, supportive care and programs for all residents. These include:

24-hour nursing care provided by Registered Nurses, Registered Practical Nurses and Personal Support Workers.
Dedicated Medical Director, Physicians, Infection Control Practitioner and Registered Dietitian on site.
Residents are encouraged to maintain mobility and quality of life through various exercises and group therapy programs, led by an Occupational Therapist, Physiotherapist and Adjuvants.
Social and therapeutic programming (such as crafts, cards, bingo, church services, special occasion and theme parties) are delivered by Recreation Services.
Counselling and psychosocial support are provided to residents and caregivers through a Social Worker.
Nutritious meals with seasonal menus.
Multi-faith space.

Rates

We offer basic rooms, semi-private rooms and private rooms. Subsidized rates may be offered for basic rooms, based on the resident's annual income.

Accommodation rates are set each July by the Ministry of Health and Long-Term Care.

Emergency Plans

Being Prepared for Emergencies

An emergency is an urgent situation or condition threatening the health or well-being of residents, staff, volunteers, and visitors to the Long-Term Care home or Adult Day Program. Emergencies like these require immediate action to ensure everyone’s safety.

The Region of Durham Long-Term Care and Services for Seniors Division has plans (codes) and processes to identify and respond to emergencies. Emergency Plans and colour-coded laminated Emergency code systems are posted in each Home and Adult Day Program. The Code System is used as a teaching, review and reference tool for staff in the event of an emergency. The emergency plans identify the roles and responsibilities for staff, visitors and stakeholders.

Testing of Emergency Codes is completed at the intervals requested by the Ministry of Long-Term Care and are outlined in each plan.

Other Emergency Plans

Long-Term Care and Services for Seniors, in consultation with Social Services Emergency Management and Durham Emergency Management, has developed and implemented the following plans for:

Communicable Disease Emergencies which are responded to using internal Outbreak Management policies and procedures which includes a Pandemic Plan
Boil Water Advisories which are activated by Public Health and responded to as per Public Health directions,
Loss of Essential Services (water, hydro, generator back-up) are responded to using applicable emergency response procedures and applicable business continuity plans
Heat Related Prevention and Management.

All plans are reviewed and evaluated annually or within 30 days of an emergency being declared over.

All staff have received training at hire and annually on the emergency plans and protocols and some staff have pre-determined roles and responsibilities during an emergency.

A RAVE system of communication will be used if staff and substitute decision makers (SDM) need to be informed on any aspect of an emergency.

Caring Connections Durham

We are excited to share the latest from Caring Connections Durham (CCD), our emotion-based model of care that is bringing residents, families, and staff closer together. Since launching in January 2024 at Lakeview Manor, CCD has been transforming everyday care into meaningful connections that truly matter.

Caring Connections Durham is an innovative, emotion-based model of care designed to strengthen meaningful connections among residents, families, caregivers, and staff. Our core principles focus on relational care and emotional well-being, emphasizing the importance of relationships in delivering high-quality care.

Key Achievements

Some highlights from our journey so far include:

Relational Care: Staff are getting to know residents' life stories more, creating intentional moments of connection, turning everyday routines into opportunities for warmth, conversation, and care.
Enhancing Experiences: Small but meaningful changes to the environment, dining, and activities are making a big difference in resident engagement and enjoyment.
Positive Outcomes: Early feedback shows increased resident satisfaction, stronger staff engagement, and a more supportive community throughout the home.
Growing Skills: Training and coaching in empathy and emotional intelligence are equipping staff and leaders to sustain these meaningful connections long-term.

Looking Ahead

CCD is now expanding across the division, beginning at Hillsdale Terraces in March 2026. We are also completing a post-pilot evaluation with Ontario Tech University, with results available in Spring 2026.

Consent & Capacity e-learning module and brochure for families

Ensuring that resident rights are respected and upheld is a fundamental principle in Long-Term Care settings.

In February 2023, a dedicated interdisciplinary team from our four Long-Term Care homes, working alongside our Legal department, started a journey to create tools, resources and education relating to resident rights and specifically to Consent and Capacity legislation. Direct care staff in our homes started to use these new and enhanced tools as of July 2, 2024.

It is important that all staff, families, and visitors, are aware of the key concepts of Consent and Capacity in Long-Term Care settings and continue to emphasize the rights of residents to make their own choices.

We are very pleased to offer an educational module to families that walks through some key concepts and pieces of legislation to assist when supporting residents. The e-learning module can be accessed here: Consent & Capacity module

A brochure was created to assist families and Substitute Decision Makers in understanding the legislation related to Consent and Capacity. A PDF copy can be found here, with printed copies available in the homes.

Connection

The Durham Region Health Department has recently launched a campaign on Connection. The campaign materials direct Durham residents to visit durham.ca/connection where they can learn more and find supportive tools to help foster connection in their lives.

The campaign highlights layers of connection such as: connection to self, connection to others, and connection to community and provides information on how this supports mental and physical health.

Family Councils

A Family Council is a group made up of family and friends of residents who live in a long-term care home. The idea is for this group to have regular meetings, be self-led, democratic and driven by the goals of the particular Family Council.

What is the purpose of a Family Council?

The purpose of a Family Council is to improve the quality of life for residents and give family and friends a forum to share their experiences, support each other and to learn and exchange information. Family Council also aids in communication between the home and the families and friends of residents.

What are the goals of a Family Council?

The goals and direction of a Family Council vary from home to home. Some examples are:

To welcome and support new families and friends.
To serve in an advisory capacity to the home.
To provide educational opportunities and sponsorship activities.
To advocate on behalf of residents and families.
To share ideas, in an effort to problem solve.
To connect with other Family Councils through networks.

Why should I join the Family Council?

Family Councils benefit their membership through support and education. It is a forum to have concerns and ideas presented, so positive changes can occur within the home. Family Councils have been very successful in making changes that benefit all the residents of a home.

Joining Family Council also

keeps members informed of what is happening in the home
provides an opportunity to voice concerns and advocate for residents
provides an opportunity to give input into decisions and positive changes in the home
allows for an opportunity to meet other families who can provide support and understanding based on their lived experiences
