Homeless support hubs serve as centralized locations designed to deliver street outreach services to individuals experiencing homelessness. By consolidating multiple resources in one place, these hubs facilitate easier access to a range of supports such as income assistance, employment services, housing outreach, healthcare, and other essential programs. This integrated approach helps maintain continuous engagement with available services, improving outcomes for people facing homelessness.

Outreach Programs Available in Durham Region

Street Outreach teams, in collaboration with partner organizations, deliver comprehensive outreach and housing-related follow-up services aimed at assisting individuals experiencing homelessness—particularly those sleeping outdoors—in securing and maintaining stable housing. Their efforts include direct engagement with people living in encampments, offering personalized support and resources to address immediate needs. Additionally, these teams facilitate access to Durham Region’s By-Name List, a coordinated housing registry that helps prioritize and match individuals with appropriate housing opportunities, streamlining the path toward long-term housing stability.
